About the role

As a Triage Team Member, you will be the first point of contact for referrers to assess each youth’s needs, conduct triages and risk assessments, and connect them to the most suitable Impart program or external support. You will liaise with stakeholders to gather key information, recommend program matches, and coordinate internal transfers or external referrals when necessary.

Commitment

● 1-3 hours/week
● 6 month commitment

Responsibilities

● Conduct triages for referrals of Youth Facing Adversity (YFA) to assess suitability for Impart’s academic programs.
● Provide recommendations and match referrals to specific programs within and beyond Impart.
● Conduct risk assessments and determine if internal transfers are required to Impart’s Mental Health Care Arm, Community Arm, or external organizations.
● Liaise with stakeholders such as social workers, case workers, case managers, SWOs, counselors, parents, and youths to gather relevant information.
● Direct referrals to external organizations for academic support when Impart is unable to meet specific needs, and follow through with the redirection process.
